Care Management Agency

Community Wellness Partners’ Care Management Agency helps individuals with high-risk and high-need conditions access care and services they need to maintain their highest quality of life.

Our experienced team of healthcare and service providers work with Medicaid or Medicaid-managed care recipients to help them identify the services available.

Care management is available to Medicaid patients in Cayuga, Herkimer, Jefferson, Madison, Oneida, and St. Lawrence counties, including those individuals who have been diagnosed with complex combinations of chronic conditions and/or severe mental illness.

Medicaid covers the cost of care management for those who are eligible, and it is free to enroll.

Care Management offers:

-Assistance to better utilize your health care, social services and other support services.

-Experienced care managers who are trained to identify and help manage medical needs and appointments, and help individuals access other services related to food, housing, rehabilitation, and legal issues.

-Assistance during a hospitalization to ensure all your needs are met at time of discharge.

Care Managers help you:

-Identify your goals and needs

-Develop a Care Plan that is shared with your primary care doctor, other physicians, mental health counselors, and service providers.

-Quickly access services to support needs and aid in recovery.
